If you have changed your name by deed pole, you then need to apply for a new passport, and send with the application form, documentation supporting the name change.

That's correct, once you change your name, your passport has to be changed to match, they have a form for it specifically, you can grab one at any library or post office, but if you try and go with your old one and get caught, it will lead them to believe it's a fake, they will screen you and you will be spending a lot of time answering questions of why your name on your ticket does not match your passport!
