I know of no country in the Caribbean where a Brit needs a visa which has to be applied for in advance. Obviously you'll have to fill in immigration and customs forms before landing, but that has nothing to do with the visa waver programme. If you are fortunate enough to have a direct flight, then you won't be subjected to the arduous routine at US airports even for people in transit.

I do not believe that you need a visa to travel to Barbados. We once belong to the British. Once before you could just use an ID but I don't know if that has changed. I can only speak for here but I think it is so with all the English-speaking islands. For the French ones I had to get a visa as well as for Puerto Rica a US visa. Below you can check out the British embassy and e-mail them for any further information.
