in canada is there a form you have to fill out if you are just going on vacation with mom and the 2 son and were can i get this form

Do you mean travelling with minor children and the fathe ris not present? If so, this is fine to do. If you have any doubts, get dad to write a note and have it notorized but no one is going to care as long as proper travel documents are in order - passports etc

They hand out a form on the plane. You might need a visa in advance.
